The Access Program provides personalized assistance to older adults and individuals with disabilities, helping them navigate essential benefits, services, and community resources. With a focus on one-on-one support, the program empowers individuals to make informed decisions that enhance their independence and quality of life.

Services Overview

The Access Program ensures that older adults and individuals with disabilities receive the guidance they need to connect with essential services and benefits. Many individuals face complex eligibility requirements, paperwork challenges, and difficulties understanding available support programs. Our trained specialists offer personalized assistance to help clients overcome these barriers, ensuring they receive the care and resources they need.

Through Information & Assistance (I&A), we provide direct, in-person or virtual support to help clients understand their options and apply for services, whether for healthcare, transportation, housing, or financial assistance. Options Counseling offers a structured approach to exploring long-term care choices, ensuring that individuals and their families have the tools to make well-informed decisions tailored to their needs.

Additionally, we administer the Benefit Access Program (BAA), which assists individuals in applying for state-funded benefits such as reduced-fare transit services, free vehicle registration discounts, and other financial relief programs. Our Access Program also supports benefit counseling and application assistance through the Senior Health Insurance Program (SHIP), the Senior Health Assistance Program (SHAP), and the Medicare Improvements for Patients and Providers Act (MIPPA), helping individuals navigate Medicare Part D, Medicare Savings Programs, and Extra Help eligibility.
